Hospice Care

The Pavilion Rehabilitation and Nursing Center's inpatient Hospice Care Program delivers comprehensive care to people who are nearing the end of their lives and offers support to family members throughout the Cape Cod area. Hospice professionals often say that their focus is on caring for the individual and not curing the disease. The goal of our program is to provide the highest quality of life during whatever time remains in the patient's life. Our services range from pain and symptom management to special therapies as needed depending on your loved one's condition.

Our Hospice Care Team

Hospice care is provided by physician-directed team of skilled, compassionate healthcare professionals and well trained volunteers who are sensitive to the special needs of our patients. They also provide families with the support they need during their most difficult times. Patients in the Pavilion's Hospice Care Program have access to a wide range of services, care methods and resources including:

 

  • On-call RN 24-hours a day, seven days a week.

  • Palliative care to manage pain and control symptoms

  • Supportive care for family members and caregivers

  • Emotional care to help achieve the highest quality of life

  • Spiritual care with visits from Clergy and Pastoral Councilors

  • Bereavement counseling for family after the death of a loved one
